2018 - 2019 PIAA Sports Physical Examinations

*** This is the ONLY physical date for the 2018 - 2019 School Year ***

When: Monday, June 4, 2018 from 6:00 PM - 8:00 PM (By Appointment ONLY)

Where: Jr/Sr High Main Office/Nursing Office

Cost: $15.00 - Please make check payable to Tulpehocken Athletic Fund

(Physical Fee Waivers are available for students who receive free or reduced lunch.)

Please check-in 5 minutes prior to your scheduled appointment.

*** All athletes attending physicals are asked to wear athletics shorts and a t-shirt and/or tank top.***

Please note that the Physicians are unable to complete Non-PIAA Physical Forms (i.e. Summer Camp, Learners Permits).

*** Additional appointment times will be added once most of the time slots are filled. Please note that we are trying to make sure appointments are evenly distributed throughout the time frame to ensure minimal wait time.
